CLERIVY — Consumer Health Data Privacy Policy

This Consumer Health Data Privacy Policy applies to "consumer health data," as defined in Washington's My Health My Data Act and similar laws in other states such as Nevada, that GUHADA Co., Ltd., operator of the Clerivy brand ("Clerivy," "we," or "us"), collects through clerivy.com and our communications. It supplements our Privacy Policy.
Consumer health data we collect
We collect consumer health data only when you choose to share it with us, for example when you tell us about a skin condition, a symptom, or a reaction to one of our products by email or through a form on the Site, including any photos you send. Before we ask for details about your health, symptoms, or photos, we explain what we collect, why, who receives it, and how to withdraw your consent, and we ask for your consent. Where the law requires it, we ask for separate consent before we share this data.

How we use it
We use consumer health data only to respond to you, to provide a replacement or refund you ask for, to review the safety of our products, and to meet legal reporting and record-keeping obligations.
Browsing and purchases
We do not use Site analytics or advertising technologies to identify you as having a health condition, and we do not build advertising audiences based on health conditions.
Your rights
You may confirm whether we collect, share, or sell your consumer health data; access that data, including a list of the third parties with which we shared it and how to contact them; withdraw your consent; and ask us to delete it. To make a request, email info@clerivy.com with the subject "Health Data Request," or write to us at the address below. We respond within 45 days, and we may extend that period once by 45 days where reasonably necessary, in which case we will tell you why. If we decline your request, you may appeal by replying to our decision with the subject "Privacy Appeal." If we deny your appeal, you may contact the Washington State Office of the Attorney General (www.atg.wa.gov) or the attorney general of your state.
